Corporate Operations Assistant

Under the direction of the Digital Asset Specialist, the Corporate Operations Assistant supports Wilshire’s strategic objectives by carrying out various activities to elevate the organization. This includes coordinating initiatives that extend our mission’s reach and establish as a leader in Home Health, Hospice, and wellness in general. The Corporate Operations Assistant reports directly to the Digital Asset Specialist and is expected to work collaboratively with their colleagues at the Agency/Program level to understand and achieve the specific financial, fund development, and technology goals.

Qualifications & Requirements

-At least two years of experience in healthcare, nonprofit communications, media relations, project management, executive support, or a related field with transferrable experiences

-Proficient in MSOffice Applications and Donor Perfect experience or donor database preferred. Intermediate Excel skills required.

-Strong detailed-oriented work habits and organizational skills are required. Able to self-start tasks and maintain workload between different assignments.

-Ability to analyze critical concepts and convey them in clearly understandable terms for multiple audiences.

-Bachelor’s Degree preferred.

Wilshire Hospice Positions

Be a part of a comprehensive and compassionate team of professionals. Our clinical team works together with physicians, nurses, therapists, musicians and counselors meeting the needs of our patients and their families.

Hospice Volunteer

Have you ever wondered what it would be like to volunteer for Hospice? Wilshire Hospice is looking for caring individuals to be Hospice In-Home Volunteers. Hospice provides the volunteer with many ways to support patients and families and opportunities for rich, meaningful relationships and experiences. Hospice Volunteers offer non-medical, practical and emotional support as well as companionship. No experience is necessary and volunteers need only bring their heart and a willingness to be of service.

Volunteer Scope
Volunteers are offered to patients and families as part of the Medicare Hospice Benefit. There are a wide range of services volunteers may offer to patients and caregivers including emotional support, caregiver relief, practical support such as errands and tasks around the home, and companionship. The presence of a volunteer in the home or facility can facilitate communication with family or caregiving staff, validate the support of caregivers and provide much appreciated companionship and gentle, skilled listening. In this time of heightened awareness of infectious diseases, volunteers are also needed to engage in supportive calls to patients, families and caregivers. These calls provide additional links to the Hospice team and may be done from home.

Weekly Commitment
Volunteers usually offer between 2 and 4 hours per week depending on their availability and the needs of their client/family. Twenty hours of training are required that include online instruction and working with the Wilshire Hospice Manual for Volunteers (provided at no cost).

Volunteer Needs
Volunteers are needed in all of San Luis Obispo County and North Santa Barbara County.
Spanish-speaking volunteers are also needed in many areas of our two Counties.

Our Wilshire Hospice Volunteer Training is designed around providing a soulful opportunity for personal growth while equipping you with the skills and support you will need for a meaningful volunteer experience.

*During this time of COVID-19 and heightened awareness of infectious disease protocol, every effort is being made to supply our volunteers with the tools and education they need to volunteer safely in our community. Why Wilshire Hospice?
Serving San Luis Obispo County since 1998 Wilshire Hospice is a local not for profit Hospice that offers a comprehensive In-Home Hospice program to terminally ill individuals and their families: including visits from nurses, social workers, spiritual counselors, therapeutic musicians, home health aides, dietitians, grief and bereavement counselors and volunteers. For 22 years, Wilshire Hospice’s goal has been to provide our community with all the things that terminally ill patients and their families need most: freedom from pain, emotional and spiritual support and the ability to control the direction of their own care.

Hospice / Home Health Aide

Under the direction and supervision of the Clinical Supervisor or designee, the Home Health Aide performs tasks that involve specific services for the patients, which are supportive and complementary to nursing and therapy practices. Core performance requirements include:

1. Patient care plans are completed based on the specific direction, without any deviation.

2. Supervised visits with the CM are completed on a timely basis.

3. Adheres to applicable State and Federal regulations governing home health, and Agency policies and procedures.

4. Communication in a professional and courteous manner with patients/families, colleagues, and supervisors, along with having no serious patient complaints reported in this past year.

5. Displays a proactive attitude and is responsive to office needs, i.e. helps others as requested by the Administrator, and functions as an active member of the team.

6. Demonstrates effective problem solving, good judgment, and confidential handling of sensitive information.

7. Completes documentation per the agency policy.

8. Completes all required CEU’s per the agency policy.

Qualifications/Requirements

-Graduation from high school or equivalent.

-Current certification by the California Department of Health Services as a Certified Home Health Aide.

-Previous experience as a Certified Nursing Assistant and/or Home Health Aide is preferred.

-Current CPR card required.

Hospice Clinical Manager

Under the direction of the Director of Patient Care Services, the Clinical Manager monitors/assures, supervises and directs field staff & Clinical Office Staff activities; is responsible for field staff competencies, and their safe and efficient comprehensive end of life care delivery to patients. The Clinical Manager is responsible for nurses, aides, social workers, physical therapist, occupational therapist, speech therapist and dietitians. Additionally, they supervise the work of contracted personnel such as therapists.

Qualifications/Requirements:

-Graduation from an approved school of nursing, BSN preferred.

-Current licensure by the state of California as a registered nurse with at least 2 years experience in home health/hospice or community health within the last 5 years.

-Demonstrated knowledge of and ability to apply advanced principles of nursing care.

-Demonstrated leadership ability, teaching skills, and the capability to work effective with all levels of personnel.

-Excellent interpersonal communication skills.

-Attentive to detail.

-Ability to problem-solve independently.

-Excellent analytical and logical skills.

-Ability to write reports and correspondence at a professional level.

-Ability to use a computer and software proficiently.

-Possess knowledge of self-care practices, including balanced health style.

Registered Nurse

Under the direction and supervision of the Clinical Supervisor, the Registered Nurse is responsible for the comprehensive nursing care of assigned patients which includes recognition of emotional, social and cultural factors, encouragement of self‐respect and self‐esteem, guidance toward rational and socially acceptable behavior and implementation of restorative nursing care measures; and for supervision and training of nursing personnel who assist in provision of care to the assigned patients. Full‐time Registered Nurse will act as Case Manager for specific assigned cases, coordinating patient care, and collaborating with other disciplines involved in the patient’s care.

QUALIFICATIONS & REQUIREMENTS

-Graduation from an approved school of professional nursing. BSN preferred

-Current licensure as a Registered Nurse in the State of California.

-Demonstrated knowledge of and ability to apply basic principles of nursing care.

-Experience and/or education in the area of the aged, chronically ill, restorative nursing, and/or psychiatric nursing.

-One or more years experience as a Registered Nurse is required; Med‐Surg experience preferred.

-Current venipuncture experience and IV skills.

-Current CPR card required.

 

Wilshire Home Health Positions

Join our home health care team, whose comprehensive care allow healing to take place in the home. Lead by our Medical Director our team of RN nurses, therapists and aides work together to support patients and families on the Central Coast.

Physical Therapist (Full Time)

Wilshire Home Health currently has an opening for a Physical Therapist. We offer independence and flexible schedules, competitive pay and mileage reimbursement; all while loving where you work!

Under the direction of the Director of Patient Care Services and/or Rehabilitation Supervisor, the Physical Therapist develops and implements a plan for providing physical rehabilitation and home therapeutic treatment, as well as education of patients, family members, and significant others.

Qualifications/Requirements
– Current licensure as a Physical Therapist in the State of California.
– Graduation from a physical therapy curriculum approved by the American Physical Therapy Association.
– A minimum of one year’s experience as a Physical Therapist is required. Experience as a Physical Therapist in home health is
preferred.
– Current CPR card required.

Informatics & Education Coordinator (Part-Time)

Under the direction of the Director of Compliance and Quality Services, the Informatics & Education Coordinator is responsible for managing development, implementation, and training for clinical software programs. In coordination with the agency and the Director of Compliance and Quality Services plans and organizes all aspects of clinical education/orientation for both Home Health and Hospice.

Qualifications/Requirements
1. Bachelor’s Degree in nursing or health services field required, Certified Nurse Educator (CNE) preferred.
2. Demonstrated knowledge of regulations governing Home Health and Hospice.
3. Demonstrated knowledge of electronic health information management practices and standards.
4. Minimum of five (5) years of clinical work experience (includes teaching, application of clinical quality improvement and principles).
5. Must demonstrate the ability to communicate effectively with Wilshire Health & Community Services (WHCS) staff and have excellent verbal and written skills, and be able to clearly anticipate ideas with poise and professionalism.
6. Must demonstrate ability to work independently and to analyze, synthesize, and organize information/data, and provide accurate and timely reports.
7. Must possess HIS and OASIS experience.
8. Netsmart software experience preferred. 

Registered Dietitian

Under the direction of the Director of Patient Care Services, the Dietitian provides diet counseling services to Home Health and Hospice patients in their home. The dietitian coordinates with other Hospice and Home Health staff to provide an efficient integration of care.

Qualifications/Requirements:

-Registered or eligible for registration as a dietitian by the American Dietetic Association.

-One year experience as a Dietitian preferred, experience in home health care/hospice preferred.

-CPR certification required.

-Excellent interpersonal communication skills.

-Attentive to details.

-Ability to problem-solve independently.

-Excellent analytical and logical skills.

-Ability to write reports and correspondence at a professional level.

-Ability to use a computer and software proficiently.

-Possesses knowledge of self-care practices, including a balanced health style.

Physical Therapist Assistant ( Full Time)

Under the direction of Rehabilitation Supervisor and/or Physical Therapist, the Physical Therapy Assistant provides home therapeutic treatment as well as education of patients, family members and significant others. This position also coordinates with other home health staff to provide an efficient integration of care.

QUALIFICATIONS & REQUIREMENTS

-Current licensure as a Physical Therapy Assistant in the State of California.

-Graduation from a physical therapy assistant curriculum approved by the American Physical Therapy Association.

-A minimum of one year’s experience as a Physical Therapy Assistant is required. Experience as a Physical Therapy Assistant in home health is preferred.

-Current CPR card required.

Central Intake Coordinator

Under the direction and supervision of the Central Intake Manager, the Central Intake –Clerical is responsible for recording and accepting referrals, determining the level of care necessary, verifying insurance coverage and acting as a liaison with referral sources and third party payors.

QUALIFICATIONS & REQUIREMENTS

Demonstrate ability to use computer programs.

Knowledgeable regarding reimbursement mechanisms – Medicare, Medi-Cal and private pay.

Demonstrate an interest and ability to work effectively with the community.

Home Health and Hospice experience is preferred.

Registered Nurse

Wilshire Community Services Positions

Be a part of our Wilshire Community Services family. As a team of coordinators, managers, and clinicians we work together as a family to serve older adults in San Luis Obispo County.

Behavioral Health Clinician

The Older Adult Full Services Partnership (FSP) Team at Wilshire Community Services provides an extensive intervention team for older adults (age 60+) with s serious mental illness. FSP clients participate in efforts to connect to appropriate health and community services for mental health treatment, physical health treatment, food and clothing assistance, transportation assistance, drug and alcohol recovery, housing and vocational services. FSP programs have adapted a “Whatever it takes” approach to promoting and encouraging wellness for enrolled FSP clients.

QUALIFICATIONS & REQUIREMENTS

1. Master’s Degree in a mental health field; CA health license or BBS inter number is required.

2. Ability to handle intense situations, display good judgment and decision making skills, and remain calm and clearheaded under pressure.

3. Demonstrated knowledge of excellent verbal and written communication skills.

Join our Creative Mediation family, as we continue to help make San Luis Obispo County a better place to live as your local community mediation center. We are a team of expert mediators and training professionals, who provide a suite of conflict resolution services to the community.

Volunteer Mediator

Join our team of volunteer mediators. This in-depth and dedicated volunteer opportunity allows our community to be a part of the mediation process.

Volunteers complete comprehensive training, advanced training, observations and practical evaluations over a 6 month period of time.
